Please complete and send this form to the IHSGB Studbook Office no later than the end of the year following the year of service, with the fee of £30 if a live birth is to be registered. Cheques should be made payable to IHSGB Ltd.
A Passport MUST be issued within 6 months of Birth or end of year of birth, whichever is the later. If unable to comply with all of the above requirements (including a microchip) to meet this deadline contact Studbook Office for advice.
